IMPORTANT: Parents have an option to potentially stay at the Francisco Grande Hotel on site during the camp and they must first request to be placed on our wait list by checking the box below. If there is room availability during the selected camp period, parents will be notified two weeks prior to the first day of camp. 

IMPORTANT NOTE: After you have completed the registration process you will be asked to submit the following documents:

1. Physician's Report, Questionnaire and Clearance Report

2. Medical Insurance Information

3. Consent for Medical Treatment

4. Heat Illness Treatment Authorization Form

5. Mild Traumatic Brain Injury Form

6. SCAS Form

7. Copy of player's insurance card Front and back

8. Copy of parent's Photo ID

9. Player’s Headshot

All the forms will be sent to you in the confirmation email with further instructions. You will also have an option to download them on the confirmation page once you have made the payment below. These forms are required to complete the registration.
